Job Description
Job Summary: The Director-Case Management ensures the provision of quality case management and Social Services and the provision of quality clinical documentation improvement.  Demonstrates principled and adept leadership of a diverse team of health-care professionals with varied degrees and education levels. Performs administrative, human resource, and financial management of the department.  Ensures departmental operations are aligned with the goals, objectives, and key Initiatives of the organization's strategic plan. Collaborates with leadership from  physician groups, service lines, Patient Care Services,  Patient Business Office, and Decision Support Services to skillfully  plan, develop,  implement, and maintain a case management, care  coordination and clinical documentation  improvement  model that  is patient and family centered  and cost effective. Establishes specific performance measures congruent with industry best practices and standards to measure quality clinical outcomes, quality clinical documentation, utilization of services, patient access and throughput, discharge planning, and cost and reimbursement.  Mentors and coaches staff in transforming  culture, processes, and care management and clinical documentation  improvement  practice to meet the demands  of cutting edge care management,  clinical documentation,  through-put  and healthcare delivery across the continuum  of care. Utilizes Interpersonal skills to maximize customer service in a manner that supports excellence in customer service, safe patient care, and professionalism.  Creates a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed.


Education and Experience: Bachelor's Degree in Nursing, Health Administration, or Social Work required. Master's Degree in health related field preferred. Minimum three years experience in the acute care setting which has provided opportunity to demonstrate competencies in care management, leadership, administration, education, and staff development required. Minimum one year experience in healthcare law and finance required.


Knowledge and Skills: Able to read; write and speak with professional quality; use computer and software programs necessary to the position (e.g., Word, Excel, Power Point, Access); operate and troubleshoot basic office equipment required for the position. Able to relate and communicate positively, effectively, and professionally with others; provide leadership; be assertive and consistent in enforcing policies ensuring compliance with regulatory standards, rules and laws; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to  communicate effectively in person, in writing, and on the telephone; think critically; work independently; perform basic math and statistical functions; manage multiple assignments; compose written material; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone, and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.


Licensures and Certifications: Current California license in Clinical Social Work (LCSW) or California Registered Nurse (RN) license required.
